Registration

Outpatients arriving at Memorial Hospital are asked to register or pre-register before their procedure.

If you know in advance that you are coming to Memorial Hospital for a service such as a planned elective surgery or the birth of a baby, you can be pre-admitted. This allows our staff to prepare for your arrival and will make for a smoother check-in on the day you arrive.

You can pre-register, register, or be pre-admitted by stopping by the registration desk Monday through Friday 6:30 a.m. – 6 p.m. Please bring your insurance card(s) with you when you are pre-registering, registering, or being pre-admitted.

Comprehensive Pain Solutions patients must register at the Comprehensive Pain Solutions office.

Scheduling

Hospital tests, rehabilitation appointments and surgical procedures must be scheduled prior to the date of service. To schedule an appointment, you will need a physician’s order for that specific service, your insurance card(s), and your current information (new address, phone number, etc.).

To schedule a test or surgical procedure, stop by the scheduling office close to the outpatient waiting area on the main floor of Memorial Hospital, or call (785) 263-6660. Scheduling hours are Monday through Friday 6:30 a.m. – 6 p.m.

To schedule rehabilitation appointments, please call (785) 263-6664 or stop by Memorial Health System Rehabilitation and Sports Medicine, which is located on the lower level of Memorial Hospital. MHS Rehabilitation and Sports Medicine hours are Monday through Friday 8 a.m. – 4 p.m.

Parking

Memorial Hospital has ample parking in front of the building. If you are handicapped or are assisting a handicapped patient, please use the handicapped parking spaces or temporarily park in the handicapped patient drop-off area at the main entrance. Wheelchairs are available inside our main entrance for anyone who has trouble walking.  Patients visiting Comprehensive Pain Solutions or the ROSE Unit may choose to park on the north side of the complex and enter through the North Entrance door.  There is also some parking available on the east side of the building.
